Step 1 - Gather documents

As a new patient, it helps us if you bring one photo ID and one proof of address.

Photo ID examples:

• Passport
• Full UK Photo Driving License

Proof of address examples:

• Council Tax Bill
• Water Bill
• Electricity/Gas Bill
• Tenancy Agreement
• Bank Statement (Original)

We can still register you without proof of address.

Step 3 - Bring to surgery

Come to the surgery with the completed GMS1 form. We’ll provide you with a registration form to complete before welcoming you to join us.
