Announcement:
We are proposing to change our trading name from Dr. Chibber’s Practice to Salisbury Avenue Surgery from Q1 2026 if approved.
- The Reason: as part of a modernising initiative, to better reflect our partnership
- Reassurance of Continuity: There is no need to re-register. All your medical records and appointments will automatically transfer to the new name.
- Changes to Contact Details: Phone numbers, email addresses and URLs will remain the same.
- NHS App/Patient Access Info: Digital services may temporarily show the old name, but login details remain valid.
Actionable Patient Information e.g. medication change notifications, pre-appointment instructions, self- management instructions, safety netting advice, review deadlines etc.
- No Action Required: patients should carry on as normal and they do not need to update their records. This information will continue to show Dr. Chibber’s practice until the new name is approved and updated.
Prescriptions: prescription forms (EPS) will be updated to the new practice name once approved but this won’t affect your medication.
How to Direct Queries: patient queries about the change should be titled ‘Trading Name Change’ and emailed to drsgupta.andpartners@nhs.net or ask Reception staff who will assist you in completing your feedback.
